The Fort Collins Salud Family Health Center (Salud) is a private non-profit community health center that has served the Fort Collins community since 2002. The Fort Collins Salud provides quality, affordable, integrated primary health care services, including: family medicine, prenatal care, family planning and gynecological services, physical exams, nutrition and health education, laboratory and x-ray, pharmacy, access to dental services, chronic disease management, cancer screenings, shared medical appointments, immunizations, minor surgery, wellness and disease prevention, migrant health outreach, pharmacy, hospitalization, behavioral health, and Medicaid/CHP+ enrollment assistance. The Fort Collins Salud clinic serves a total of 10,275 patients through 30,478 patient visits.
Submissions for Mural:
The following materials will be required to complete this artist call:
1. A letter of intent explaining the artist's interest in being considered for this commission. The letter should include:
• Why the artist is interested in working with Salud.
• The artist’s experience working on similar projects in the past and how this past work is relevant to the project at Salud.
• The artist’s proposed vision for the mural. Please note: the artist should submit a written proposal for one of the three clinic spaces described above. All three options will be weighted equally by the Selection Committee.
• Optional: In addition, the artist may submit a sketch of the artist’s proposed vision for the mural.
2. Current artist resume, as well as contact information for a professional reference that can speak to the artist’s ability to complete the proposed work.
3. Three digital images of recent work—submitted as attachments (19x20, 72-120 dpi, maximum of 5MB) and an identification list including:
• Image number
• Artist name
• Title
• Year
• Medium
• Location
• Project budget and time to complete project
Salud’s proposed budget for the project is $1,500, however, as this is the first RFE, this will be a learning process for Salud. Please state your commission/ budget needs and it will be part of the review. Salud will reach out to artists to discuss the budget as needed as we do not intend for this to be a limiting factor. Salud will cover the costs of materials (preferably as a reimbursement).

Selection Process:
The Selection Committee (consisting of volunteers, community members, and clinic staff) will review submissions based on criteria. The committee will request additional information as needed. Following their review, the committee will select 3-4 finalists and schedule a meeting with each finalist to discuss the project. The artist will then create a concept for the proposed work and meet with the committee once more to present their concept. The committee will then make their final selection and the selected artist(s) will finalize their concepts.
Deadline:
The Request for Entry (RFE) will be released: March 5, 2015
Submissions are due no later than: 5:00 p.m. on March 26, 2015
The Selection Committee will announce their decision by: April 23, 2015
